We used data from an economic evaluation piggybacked to a randomised controlled trial in patients with chronic obstructive pulmonary disease COPD ; to study the impact of dropout on the cost analysis. We compared complete cases analysis, mean imputation, predicted regression, hot-decking and multiple imputation with regard to the estimation of the mean SE ; cost per patient and the difference in costs between treatment groups. This study showed that the way that is dealt with the cost data of dropouts had a large impact on the outcomes of the economic evaluation. Information about the rate of dropout and the way data of dropouts are treated are of vital importance to assess the results of economic evaluations and should always be reported. Because this study was based on empirical data in which the `true' costs were unknown, we were not able to determine which of the methods performed best. In current research we are conducting simulation studies to assess the performance of these methods in data with various distributions and different dropout patterns.
